Reliant Medical Group is seeking a Clinical RN Specialist to join our exciting and innovative team!

Additional locations covered: Metrowest Region/Framingham
Provides assessments, treatment plans, and referrals for opioid substance use disorder for patients. Interfaces with physicians, health care team, patient, family members and community resources in managing members’ treatment and aftercare plans.
Performs intervention assessments for adult and adolescent patients with opioid substance use disorder, abuse and/or addiction.
Develops treatment, recovery and aftercare plans following an evidence-based best practice for opiate substance abuse and addictions within scope of responsibility.
Creates and communicates care plans to the patient, primary care physician, behavioral health provider, social worker, case manager, pharmacy, and other members of the health care team as appropriate.
Provides referrals to community resources, and serves as an advocate for patients to find placement in inpatient treatment facility, residential detoxification or recovery program, outpatient treatment program, support group, family support group, therapy and/or counseling as appropriate to plan.
Integrates practice into primary care settings to serve as a support and resource for PCPs and other members of the health care teams in management of patients with opioid high use patterns, abuse and/or addictions disorders.
Assists individuals with developing comprehensive aftercare plans. Meets with patients for limited and pre-determined period of time for individual counseling in order to assist in attaining treatment goals. Incorporates patient self-care and shared decision making in all aspects of treatment and aftercare plan.
Tracks patient roster, manages follow-up care according to treatment plan, using in person visits and telephone calls.
Works in collaboration with our Care Managers as a source of referrals and for discharge planning form an inpatient setting
Provides direct support to family members including ad hoc telephone advice.
Implements relapse prevention counseling and guides patients to prescribers of pharmacotherapy as appropriate.
Reviews provider prescribing patterns and offers education and guidance to alter high risk prescribing.
Has knowledge of substance abuse associated mental illness, poly-substance use disorders, and pain management options and resources beyond opioids to help inform treatment and aftercare plans.
Ensures that relevant providers receive timely and confidential clinical data for care treatment decisions in all settings of care.
Knowledge of and ability to work effectively with community based resources, referral networks, and public/private systems of care.
Participates in periodic case reviews with provider groups to educate and inform prescribers about patient dependency risk, opioid substance abuse, and addiction.
Promotes cultural awareness working with a diverse population.
Assists with other addiction related projects, such as developments of screening tools, as required or directed.
Complies with established departmental policies, procedures, and objectives.
Enhances professional growth and development through educational programs, seminars, etc.
Attends a variety of meetings, conferences, and seminars as required or directed.
Regular, reliable and predicable attendance is required.
Performs other similar and related duties as required or directed.
Graduate from an accredited school of nursing. Active, unrestricted licensure as a Registered Nurse in Massachusetts. Bachelor’s degree in nursing, and Masters in healthcare or related field preferred. Certified in Addictions Nursing or licensed in Substance Abuse Counseling strongly preferred. A minimum of three years of experience in substance abuse field. Current Massachusetts driver’s license. Demonstrate excellent communication, interpersonal and organizational skills. Proficient in computer use, the Internet and health information technology. Must be flexible and adaptable. Ability to travel frequently to Reliant practice sites, community treatment facilities and other sites where patients receive care.
