Overview

Performs a variety of routine duties related to the maintenance of the medical records. Provides a variety of administrative and clerical support for all areas of the HIM department, such as scanning, abstracting, roi, warehouse and other related duties.

 

Position: Full Time 38.75 Hours per week 

Hours: Monday through Friday 8:00 AM - 4:30 PM 

 

 

Responsibilities

Retrieves paper records for clinical areas, provides documentation from paper record to support patient care, files documents, and assembles records following standard procedure.

 

Analyzes each report to determine method of storage: paper record or scanning, and proper acknowledgement prior to storage.

Identifies and investigates potential duplicate medical record numbers and consolidates electronic medical record and paper records appropriately.

 

Investigates and separates patient records that were consolidated in error.

 

Performs audits of paper records as required following standard procedures.

 

Manages paper flow between clinic and area hospitals as required.

 

Maintains equipment such as fax, printers, copy machines as required. Notifies manager or others of issues. 

 

Assists in patient requests for information release and redirect phone calls when appropriate. Explains basic authorization requirements, provides information for routine requests, and clarifies requests as directed.

 

Updates the PCP in the electronic medical record system when a release authorization has been received requesting records to be transferred to an outside primary care provider.

 

Responds to clinical staff or others’ requests for information either through redirection or searching paper or electronic records.

 

Acts as a liaison between clinic business departments and off site record storage vendor. Coordinates all box pick ups from departments, sets up / removes boxes in vendor system, and manages box requests from vendor.

 

Performs record purge and destruction functions following standard procedures.

 

Prints and mails required documentation to support clinical areas as required.

Qualifications

High school education or equivalent and up to and including one year relevant experience. Excellent customer service and communication skills. Good organizational skills and attention to detail. Ability to perform all physical activities required for the filing and retrieval of medical records and documents, including lifting up to 50 lbs. Knowledge of various software applications such as Microsoft Word, Outlook, etc.
